29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At the residential construction site located at 8700 NE 82nd Street in Liberty, Missouri:  Employees were exposed to struck by hazards. Employee was observed installing wood materials using a nail gun without eye/face protection.

Employees were observed installing materials and working at height.  Employees were exposed to a fall distance of greater than 8 feet eave to ground without use of fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the residential construction site located at 8700 NE 82nd Street in Liberty, Missouri: Employees were exposed to fall hazards. Employees were observed accessing and egressing the roof by way of a portable ladder that did not extend 3 feet past landing.  Employees were exposed to a fall distance of greater than 8 feet eave to ground without use of fall protection.
