29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about April 22, 2019, and at times prior thereto, the employer did not establish energy control program with procedures and steps to lock out and tag equipment where employees clean and maintain energized equipment daily and where third party companies perform maintenance on the equipment exposing employees to struck-by, caught-in and amputation hazards.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):   On or about April 22, 2019 and at times prior, the employer did not provide a machine guard for the operating belts and pulley's below the blue trays where employees clean and maintain the blue trays daily, exposing them to struck-by, caught-in and amputation hazards.

29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours.  On or about April 22, 2019, the employer did not provide injury and illness logs to an OSHA representative within 4 hours.  OSHA received the 300 logs on May 31, 2019.
