29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: a) On or about 04/17/19, on a roofing jobsite, at a house at 156 Wesley Ave., Buffalo, ny: A ladder Jack Scaffold was erected on the back side of the house. The scaffold had two support ladders, and the brackets that supported the aluminum pik were on the outside (west side of the ladder). Employees climbed over the brackets to get on/off the pik. A direct access ladder to the pik was not provided. NO ABATEMENT CERTIFICATION REQUIRED

Pitch of the roof was approx. 10 in 12. NO ABATEMENT CERTIFICATION REQUIRED

An extension ladder was provided for employees to access the aluminum pik on the scaffold. The side rails of that extension ladder were only approx. 31 inches above the pik. NO ABATEMENT CERTIFICATION REQUIRED
