29 CFR 1910.119(d)(3)(iii): For existing equipment designed and constructed in accordance with codes, standards, or practices no longer in general use, the employer did not determine and document that the equipment in the process was designed, maintained, inspected, tested, and operating in a safe manner:  The employer did not comply with recognized and generally accepted good engineering practice such as IIAR-2 (1992 or 2014), Section 13 .3 .1 Required Shut-Off Valve Locations and Section 15.6 Equipment and Piping Hydrostatic Overpressure Protection, when it did not ensure that equipment was installed in accordance with RAGAGEP requirement, exposing employees to loss of containment and overpressure conditions causing the hazards of inhalation of toxic ammonia and ammonia burns.  The employer did not determine and document that the equipment in the process was designed, maintained, inspected, tested, and operating in a safe manner. At the time of the inspection the following equipment did not comply with recognized practices: a) South NH3 Room by the King Valve. There is a bypass 2" line liquid HPL car-sealed closed valve before the king valve going over to the accumulator. This line should be above not below the King Valve. b) HE2/OP-2 Low (HE-2 RDA-1 250# HG27 in the South 38 System) - Heat Exchanger #2; Oil Pot #2; Rupture Disc Assembly. Employer installed a regular Pressure Relief Valve not a Liquid Relief Valve, thus not using the correct valves for the applications. c) HPR-1 High HPL Liq Ammonia High (RDA-3 250# H27 in the North 10 System) - High Pressure Receiver; High Pressure Liquid. Employer is not using the correct valves for the applications. d) HPR-2 High HPL Liq Ammonia High (RDA-5 250# HG27 in the South 38 System - High Pressure Receiver; High Pressure Liquid, not using the correct valves for the applications. e) RC-1 Innercooler (RDA-1 300# HG27 in the North 10 System) - Refrigerant Compressor, not using the correct valves for the applications. f) RC-2 Innercooler (RDA-1 300# HG27 in the North 10 System - Refrigerant Compressor, not using the correct valves for the applications.
