At approximately 94 Reid Ave., Staten Island, NY - Middle of street The employer did not ensure the location of underground utilities was determined in a safe and acceptable way. Employees engaged in the installation of water service failed to ensure accuracy of mark out locations prior to excavating and did not locate the exact location of a gas main by hand digging or other safe and acceptable means. Violation occurred on or about April 25, 2019
