29 CFR 1926.301(a):Unsafe hand tool(s) were issued or permitted to be used: Location: Ardsley Rd & Sprain Valley Rd.- Bridge Overpass a) On or about August 13th, 2022, an employee was removing and repairing concrete on an overpass bridge using the Hell Dog Rivet Buster machine. The employer permitted the use of the rivet buster machine with an improperly working piston that exposed the employee to an amputation hazard.
