29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    (a)Reyes Roofing, L.L.C. at 8804 West 18th Street, Sioux Falls, SD 57106: On or about and at times prior to April 23, 2019, the exposing employer did not ensure employees performing roofing activities on a 5-in-12 pitch roof at heights greater than 6 feet above ground level were protected from fall hazards by use of appropriate fall protection systems. This condition exposed employees to fall hazards. between 8 and 15 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)Reyes Roofing, L.L.C. at 8804 West 18th Street, Sioux Falls, SD 57106: On or about and at times prior to April 23, 2019 the exposing employer did not ensure employees performing roofing activities on a 5-in-12 pitch roof at heights greater than 6 feet above ground level were protected from fall hazards in that, the ladder used to access the roof on the gable edge of the front of the house was not extended at least 3 feet above the upper landing surface. This condition exposed employees to fall hazards.

This condition exposed employees to eye hazards.
