29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a) 1800 Centre Avenue, Reading, PA 19605 - Franklin Labs - On or about April 23, 2019 the employer did not conduct air monitoring for carbon monoxide (CO) to determine that air quality is being maintained at and below the Permissible Exposure Limit when using propane saw cutting equipment in enclosed interior spaces. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.55(a): Employee(s) were exposed to material(s) at concentrations above those specified in the Threshold Limit Values of Airborne Contaminants for 1970 of the American Conference of Governmental Industrial Hygienists: (a) 1800 Centre Avenue, Reading, PA 19605 - Franklin Labs - Three employees using a propane concrete saw inside an enclosed interior space were exposed to carbon monoxide at a calculated 8-hour time weighted average concentration of at and above 61.7 parts per million(ppm) which was 1.23 times the OSHA permissible exposure limit of 50 ppm, on or about April 23,2019. The exposure level was derived from calculations using carboxyhemoglobin blood levels measured following the exposure. Atmospheric pressure, height, weight, activity levels, tobacco smoking behavior, and other factors were taken into account. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.
