29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:        a) Ladder Access - Beam Street Facade: Employee engaged in steel erection activities was exposed to fall hazards of approximately 16 feet while utilizing an extension ladder that did not reach the top of the beam, on or about 4/30/19.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    a) 2nd Tier Deck: Employees without proper fall protection were exposed to approximately 22 foot fall hazards while engaged in steel erection by welding bridging onto steel trusses, on or about 4/30/19.    b) 1st Tier Beam: Employee without any fall protection was exposed to fall hazards of approximately 16 feet while engaged in steel erection by plumbing up the building, on or about 4/30/19.     New Jersey Iron Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.760(a)(1), which was contained in OSHA inspection number 1365359, citation number 01, item number 01 and was affirmed as a final order on 4/1/19, with respect to a workplace located at 620 Ocean Ave., Long Branch, NJ.
