Location: 346 43rd Street, Brooklyn, NY; 4th Story.    On or about 5/1/2019,    a) Employees erected a supported tubular scaffold without baseplates.  The legs of the scaffold frames pierced through the footings and the scaffold collapsed after it was loaded with masonry material.
