29 CFR 1910.119(e)(1): The employer did not perform an initial process hazard analysis (hazard evaluation) on processes covered by 29 CFR 1910.119:  a.  On or about 04/30/2019, at the Kosan Crisplant facility in Sebring, FL, an initial process hazard analysis (PHA) had not been conducted for the following covered process which contained over 10,000 pounds of propane:  1) Propane bulk storage and transfer process which included, but was not limited to, a 1000 gallon propane storage/transfer vessel and an 18377 gallon propane storage tank and auxiliary equipment including, such as, but not limited to propane piping, pumps, valves, and sensors.  2) Propane filling operations which included, but was not limited to, a propane carousel, propane sensors, conveyors, and filling equipment.  3) Propane refurbishing operations which included, but were not limited to, propane cylinder sand blasting operations and painting and scraping of cylinders.  4) Cylinder evacuation process which included, but was not limited to, pressure piping for evacuation, hoses, and evacuation stations.  5) Cylinder re-valving operation which included, but was not limited to, two valver machines.  6) Cylinder storage which included approximately 10,000 twenty-pound propane cylinders in storage around the yard/facility in various places and the operation of forklifts which could pose an ignition hazard.

29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process.   a. At the site, employees were exposed to fire and explosion hazards during cylinder refurbishing and filling operations.:  Operating procedures were not implemented and developed to ensure that leaking cylinders and/or cylinder valves were handled in a safe manner and at a designated safe location, and that the sorting of incoming cylinders to the process was handled such that any residual propane in the cylinders was detected and evacuated in a safe manner.
