Harmony Rd., Ft. Collins, CO 80528: On or before May 1, 2019, the employer had not provided audiograms to employee that were exposed at or above the action level of 85 dBA. An employee operating a punch press was exposed to noise at a level of 92 dBA.    (b) Walker Manufacturing Company, 5925 E. Harmony Rd., Ft. Collins, CO 80528: On or before May 1, 2019, the employer had not provided audiograms to employee that were exposed at or above the action level of 85 dBA. An employee welding in the deck area was exposed to a noise at a level of 90 dBA.    (c) Walker Manufacturing Company, 5925 E. Harmony Rd., Ft. Collins, CO 80528: On or before May 1, 2019, the employer had not provided audiograms to employee that were exposed at or above the action level of 85 dBA. An employee working in the grinding area was exposed to noise at a level of 85 dBA.

The employer did provide eye wash bottles, however these do not meet the requirements of ANSI Z358.1-2014.
