29 CFR 1910.178(m)(1): Trucks were driven up to person(s) standing in front of a bench or other fixed object:    On or about April 22, 2019, at Line 1, a forklift operator was in the process of moving two full Master Cartons when the cartons fell crushing an employee.  The employee had been standing between the load of Master Cartons on the forklift and an empty Master Carton in front of a fixed conveyor.

29 CFR 1910.305(g)(2)(iii):  Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws:  On or about April 30, 2019, at Line 1, employees were exposed to electrical shock hazards from a flexible cord used to power the F4 Flaring Machine with the outer insulation pulled from the plug end of the cord exposing the inner wiring and the machine was not removed from service.
