29 CFR 1910.179(b)(3): Crane(s) were modified and such modifications and the supporting structure was not checked thoroughly for the new rated load by a qualified engineer or the equipment manufacturer:    On or about October 24, 2019, employees utilized an underhung overhead bridge crane to move 600-pound loads of tubing and the employer had modified one post after an accident, but did not have the crane checked for load capacity by a qualified engineer or equipment manufacturer.

29 CFR 1910.179(j)(2): The following items were not inspected for defects at intervals as defined in paragraph (j)(1)(ii) as daily to monthly intervals, or as specifically indicated, including observation during operation for any defects which might appear between periodic inspections:     On or about October 24, 2019, employees used a bridge crane to move 600-pound rolls of copper tubing and the employer did not conduct frequent inspections of the crane to identify deficiencies including:    a) All functional operating mechanisms for maladjustment interfering with proper operation (Daily);    b) Deterioration or leakage in lines, tanks, valves, drains pumps, and other parts of air or hydraulic systems (Daily);    c) Hooks with deformation or cracks (Daily) and inspection with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected (Monthly). For hooks with cracks or having more than 15 percent in excess of normal throat opening or more than 10° twist from the plane of the unbent hook refer to paragraph (l)(3)(iii)(a) of this section;    d) Hoist chains, including end connections, for excessive wear, twist, distorted links interfering with proper function, or stretch beyond manufacturer's recommendations (Daily) and inspection with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected (Monthly); and    e) All functional operating mechanisms for excessive wear of components.

29 CFR 1910.178(l)(6): The employer did not certify that each operator had been trained and evaluated as required by paragraph (l) through a certification that included the name of the operator, the date of the training, that date of evaluation, and the identity of the person(s) performing the training or evaluation:  On or about October 24, 2019, an employee was exposed to struck-by hazards from fork trucks operated in the Warehouse and Production Areas and the employer failed to certify the operator by documenting the following:  a) name of the operator;  b) date of the fork truck training;  c) date of the fork truck evaluation; and   d) the identity of the person(s) performing the training or evaluation.
