29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedures and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):  a) Production Floor, 14090 Shell Belt Road, Bayou La Batre, AL:  On May 2, 2019 and at times prior, the employer failed to develop and implement an energy control program and training to protect employees from hazards associated with the release of stored energy such as, but not limited to, electric and kinetic, when performing service and maintenance activities on machines and systems such as, but not limited to the Ultra-Freeze Cryogenic Freezer and the Bagger Machine.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  a) Production Floor, 14090 Shell Belt Road, Bayou La Batre, AL:  On May 2, 2019 and at times prior, the employer exposed employees to struck-by and amputation hazards, in that employees were allowed perform set-up, maintenance, clearing, and cleaning operations on equipment such as, but not limited to, the bagger machines and the Ultra-Freeze Cryogenic Freezers, without developing, documenting and utilizing lockout/tagout energy control procedures to prevent accidental startup of the machine.  Employees were allowed to clear jams and clean the machines using only the E-stop and/or limit switches in lieu of locking out the machine.
