29 CFR 1926.502(d)(6)(i): A snaphook not designed for this type of connection, was engaged directly to webbing, rope or wire rope: a) 190 South Street, Morristown, NJ. Employees constructing the leading edge of the third floor of this new building were exposed to falls when they did use their fall protection lifelines as they used them improperly by anchoring the end of the linelines around floor joists by connecting the snaphooks back to the lifelines. Violation observed on or about 05/02/2019.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section: Note: Circumstances where retraining is required include, but are not limited to, situations where (1) Changes in the workplace render previous training obsolete; or (2) Changes in the types of fall protection systems or equipment to be used render previous training obsolete; and (3) Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicated that the employee has not retained the requisite understanding or skill. a) 190 South Street, Morristown, NJ and at the Corporate Offices at 63 West Chrystal Street, Dover, NJ. Employees constructing the leading edge of the third floor of this new building who had once been trained on fall protection following 1926.503(a) but as shown by being exposed to falls and using fall protection equipment incorrectly, showed inadequate understanding and skill for employees who had been trained following the OSHA fall protection training regulation. Violation observed on or about 05/02/2019.

Employees constructing the leading edge of the third floor of this new building were exposed to falls of 11 feet to the floor below or to falls of 26 feet off the side of the building as fall protection was not being used. Violation observed on or about 05/02/2019. CHT Construction, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(2)(i), which was contained in OSHA inspection number 1165730, citation number 1, item number 2, and was affirmed as a final order on 09/08/2016, with respect to a workplace located at 2 Park Place, Morristown, NJ.
