29 CFR 1910.22(a)(1): All places of employment were not kept in a clean condition:    a) On or about 05/06/19 in Plant 2 Woodworking shop; employees were exposed to potential fire hazard when wood dust and pieces of wood were piled to a height of 30 inches, 42 inches wide and 10 feet in length in the area below the Verticut 2000 saw table due to the lack of a dumpster to discard the wood coupled with the dust collector system for this saw not functioning properly.       b) On or about 05/06/19 in Plant 2 Woodworking shop; employees were exposed to potential fire hazard when wood dust was clearly visible to a depth of 1/4 inch behind the Verticut 2000 saw table, plied to a height of 3-6 inches up a support beam and piled on top of a drum of bar chain oil due to the lack of a dumpster to discard the material and the dust collector system for this saw not functioning properly.         c) On or about 05/06/19 in Plant 2 Woodworking shop; employees were exposed to a potential fire hazard when a pile of wood dust approximately 2 feet in diameter and nearly two feet high was observed on the mezzanine for the plant dust collector blower motors.        ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) On or about 05/24/19 in the Production area; during change over and set up of the vacuum former machines, employee were exposed to caught between hazards of the platens when they relied on the safety interlocked access doors for energy isolation instead of applying locks to the energy isolation devices.        b) On or about 05/24/29 in the Saw area; during the set up of the Bruno Die Cutter, employees were exposed to caught between hazards of the upper portion of the die cutter ram when they opened safety interlocked guard to access the height adjustment devices instead of applying locks to the energy isolation device as described in the Bruno Die Cutter Lock out procedure posted on the machine.          c) On or about 05/24/19 in the Production area; during the dislodging of the jams through the lower door on the foam densifiers, employees were exposed to being caught in the auger when they relied on the open safety interlocked lower door instead of applying a lock to the energy isolation device.     ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) On or about 05/24/19 near the Thermo Former; employees were exposed to fire hazard when the employer allowed the use of a two relocatable power taps daisy chained to provide power to a battery charger.  One power tap was plugged into an extension cord that was pulled into another power tap that was plugged into another extension cord that was plugged into an outlet.  The second power tap was secured to one leg of a small table with two plastic ties.      b) On or about 05/24/19 near the Bruno die cutter; employees were exposed to a fire hazard when the employer allowed the use of a relocated power tap, secured to a metal frame of a work table with two plastic ties, to provide power for a battery charger.     NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  a) On or about 05/24/19 near the Thermo Former; employees were exposed to fire hazard when the employer allowed the use of a two extension cords to daisy chain and provide power to two relocateable power taps.   An extension cord from a power tap on the work table on the east side of the machine was plugged into an extension cord which ran overhead and was snaked around a wiring control support arm of the Thermo Former, across the machine and down to another power tap where a second extension cord was used to provide power to the second power tap.    NO ABATEMENT CERTIFICATION REQUIRED
