29 CFR 1910.147(f)(3)(i): When servicing and/or maintenance is performed by a crew, craft, department or other group, they shall utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device.  a) On or about 01/31/2024 at the Saw Department, when employees cleaned the Gong Yong Splitter after the incident, a group lock-out procedure was not followed to ensure every employee involved in the cleaning process applied their own lock to the group lockout device.   b) On or about 01/31/2024 at the Saw Department, when employees changed the blade of the vertical saw, a group lock-out procedure was not followed to ensure every employee involved in the cleaning process applied their own lock to the group lockout device. 29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, caught-by points and rotating parts, :  a) On or about 01/25/2024 at the Saw Department, employees were exposed to ingoing nip points and rotating parts hazards when operating the Gong Yong Foam Splitter. While an employee was feeding a foam sheet into the machine, their hand got pulled in with the material into the machine resulting in the employee sustaining multiple finger amputations of their right hand.  ABATEMENT DOCUMENTATION REQUIRED
