29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems: a) Job site, 3290 Sawmill Run Boulevard, Pittsburgh, PA: On or about May 3, 2019, during metal stud installation activities, the employer did not ensure that employees appropriately used fall protection systems/devices at all times where a fall hazard of approximately 30 feet existed.

29 CFR 1926.760(d)(2): Fall arrest system components used in fall restraint systems did not conform to the criteria in 29 CFR 1926.502 (see Appendix G): a) Job site, 3290 Sawmill Run Boulevard, Pittsburgh, PA: On or about May 3, 2019, during metal stud installation activities, the employer did not ensure that appropriate fall protection systems/devices were in use in that personal fall arrest systems were set up in such a fashion that an employee could have fallen from the edge of the building, fallen more than 6 feet or swung and strike a beam below them.
