29 CFR 1926.300(b)(2): Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded: (a) New residential construction located on Lot #30 (front side): On or about 05/03/2019, and at times prior, the employer exposed an employee to severe laceration and amputation hazards while operating a concrete mixer, in that the affected employee had to start the mixer with a strap exposing the employee to moving parts of the starter recoil as the starter recoil cover was missing.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stair rail system along each unprotected side or edge: (a) New residential construction on Lot #30 at a stairway located between the first and second floors: On or about 05/03/2019, and at times prior, the employer exposed employees to falls of up to 12-feet, 9-inches while using an open sided stairway without a stair rail on the open side which had an opening of approximately 11 inches between the stairway and the framed wall.
