29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about May 8, 2019, employees were exposed to ingoing nip points, rotating parts and crushing hazards while working on and around the PCMC Printer. The employer did not ensure the point of operation was properly guarded. The KG Marketing and Bag Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(i), which was contained in OSHA inspection number 1150683, citation number 1, item number 1 and was affirmed as a final order on November 15, 2016, with respect to a workplace located at 1201 South Grandview Boulevard in Waukesha, WI.
