29 CFR  1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a)  On or about August 9, 2022, employees were exposed to ingoing nip points, rotating parts and crushing hazards while working on and around the PCMC Printer.  The employer did not ensure the point of operation on the back side of the PCMC Printer  was properly guarded, in-that guards were missing or not properly closed and secured around rotating spindles and gears.  (b)  On or about August 9, 2022, employees employees were exposed to ingoing nip points, rotating parts and crushing hazards while working on and around the Hudson Sharp - FMC Corp. Plastic Bag Wicket Machine - Machine #6 (Model No. 105 MX & Serial No. B1711 LH).  The employer did not ensure the point of operation created by the rotating spindle that seals the bags and stakes the bag was properly guarded.  The KG Marketing and Bag Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(i), which was contained in OSHA inspection number 1399064, citation number 1, item number 1 and was affirmed as a final order on September 23, 2019, with respect to a workplace located at 1201 South Grandview Boulevard in Waukesha, WI.
