29 CFR 1910.134(h)(1)(i) as required by 1926.103: Respirators issued for the exclusive use of an employee were not cleaned and disinfected as often as necessary to be maintained in a clean and sanitary condition:     a. I-89 Bridge over Turkey Pond, Concord, NH - On or about May 9, 2019, employees wearing 3M model 7503 half-face elastomeric respirators after blasting operations were completed were exposed to lead and arsenic containing dust on the inside of the respirator.  Wipe samples taken from the inside of the respirator contained 549 micrograms of lead and 0.73 micrograms of arsenic.  The employer failed to ensure that the respirators were cleaned as often as needed based on the site conditions.  Employees were exposed to inhalation and ingestion of lead dust.

I-89 Bridge over Turkey Pond, Concord, NH - On or about May 9, 2019, employees wearing supplied air Bullard abrasive blasting hoods were exposed to exposed to the hazards of carbon monoxide.  The Ingersoll-Rand Model 1600 air compressor that supplied air to the Bullard abrasive blasting hoods did not have a high temperature alarm or carbon monoxide alarm.

29 CFR 1926.62(i)(2)(i): The employer did not provide clean change areas for employees whose airborne exposure to lead was above the permissible exposure limit, without regard to the use of respirators:    a. I-89 Bridge over Turkey Pond, Concord, NH - On or about May 9, 2019, employee airborne exposure to lead was above the permissible exposure limit, and the clean side of the shower trailer was contaminated with lead.  Up to 359 micrograms of lead was located on locker handles and up to 83.7 micrograms per 100 square centimeters was located on the bench.  Employees were exposed to inhalation and ingestion of lead dust.

I-89 Bridge over Turkey Pond, Concord, NH - On or about May 9, 2019, the employer did not ensure that eating areas were as free as practicable from lead contamination.  Up to 75.6 micrograms per 100 square centimeters was located on coolers in the box truck eating area.  Employees were exposed to inhalation and ingestion of lead dust.

I-89 Bridge over Turkey Pond, Concord, NH - On or about May 9, 2019, abrasive blasting suits and footwear worn during lead abatement activities did not have surface lead dust removed prior to entering lunch areas.  Employees were exposed to inhalation and ingestion of lead dust.
