Hill Railroad Car Company, Inc. - On or about 5/9/19, and at times prior thereto, employees were performing servicing and maintenance to equipment/machinery including, but not limited to, welding rod oven and burning machine. The lockout/tagout procedures did not explain how to bleed off propane, and oxygen, the magnitudes of energy, how to test equipment to verify de-energization, and specific methods/steps to disconnect compressed gases.

The insulation and the wires were being pulled out of the box.
