29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:      A.)  Hill Railroad Car Company, Inc. -  On or about 5/9/19, and at times prior thereto, employees were performing servicing and maintenance to equipment/machinery including, but not limited to, welding rod oven and burning machine.  The lockout/tagout procedures did not explain how to bleed off  propane, and oxygen, the magnitudes of energy, how to test equipment to verify de-energization, and specific methods/steps to disconnect compressed gases.

Hill Railroad Car Company, Inc. -  On or about 5/9/19, and at times prior thereto, employees were performing servicing and maintenance to equipment/machinery including, but not limited to, welding rod oven and burning machine.  An annual periodic inspection of the procedures was not performed with all authorized employees.

Hill Railroad Car Company, Inc. - On or about 5/9/19, and at times prior thereto, The Lincoln arc welder did not have covers over the terminals.  The caps were missing.  The welder is used to perform repair work on railroad cars.

Hill Railroad Car Company, Inc. - On or about 5/9/19, and at times prior thereto, the Lincoln arc welder had exposed wires where the insulation had been pulled away from the terminal area.  The welder is used to perform repairs on railroad cars.

29 CFR 1910.305(b)(1)(iii): Cable was not secured to the cabinet, cutout box, or meter socket enclosure:      A.)  Hill Railroad Car Company, Inc. - On or about 5/9/19, and at times prior thereto, the cable coming out of the 480 volt 3 phase disconnect for a welder was not secured to the box.  The insulation and the wires were being pulled out of the box.
