29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit".  On or about 8 May 2019, and at times prior thereto, exits for the facility were not clearly visible and marked within the warehouse floor, exposing employees to the various hazards of a fire. A) Unmarked Exit door along the west wall inside the warehouse. B) Unmarked Exit door along the west wall inside the warehouse. C) Exit sign not visible along the west wall and other portions within the warehouse.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.  On or about 8 May 2019, and at times prior thereto, a charging station for the Tennant 5700 was not equipped with an eye wash station for immediate emergency use, exposing the employees charging and adding distilled water to the hazard of being struck by corrosive liquids and materials.

29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting.  On or about 8 May 2019, and at times prior thereto, employees were not trained on the general principles of the fire extinguisher use and the hazards involved with incipient stage fire fighting, exposing employees to the hazards involved with fires.

29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel.  On or about 8 May 2019, and at times prior thereto, two disconnect switches within the warehouse were blocked and not readily accessible, exposing employees to a tripping hazard. A) Disconnect switch at the Damaged/Written Off Items area. B) Disconnect switch at dock bay door.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval.  On or about 8 May 2019, and at times prior thereto, a man-basket was being used with a propane forklift inside a warehouse in order to gain access to higher elevations without the written approval of the forklift manufacturer, exposing the employee using the basket to the hazard of falling over 20 feet to the concrete slab below.
