29 CFR 1926.501(b)(4): Each employee on walking/working surfaces was not protected from falling through holes, more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a) 1912 East York Street, Philadelphia, PA: Floor holes exposed employees to fall hazards of approximately 10 feet. The employer did not properly cover the holes or provide a fall protection system to protect employees.  Condition was observed on or about 7 MAY 2019.
