Employees were working on a four-story flat roof condominium approximately thirty (30) feet from the ground demolishing a ten (10) foot high penthouse and loading debris onto a lull at the roof's edge while not wearing personal fall protection and were not trained to recognize the hazards of falling and the procedures to be followed in order to minimize the fall hazards.
