Employees were re-roofing a commercial building golf clubhouse. The employees were throwing shingles, nails, and roofing materials to the ground and/or lower level while other employees were on the lower level or ground. The employer had not provided protective helmets. This condition exposed approximately 4 employees to struck-by hazards.

Employees were re-roofing a commercial building golf clubhouse. The employees were throwing shingles, nails, and roofing materials to the ground and/or lower level while other employees were on the lower level or ground. Employees were using pneumatic nail guns while shingling, and hammers to remove or replace roofing materials. The employer did not ensure that each affected employee uses appropriate eye or face protection. This condition exposed 8 employees to approximate struck-by hazards.

Cobra Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1342687, citation number 1, item number 1, and was affirmed as a final order on December 28, 2018, with respect to a workplace located at 5080 S Irving St. Englewood, CO. Cobra Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1189578, citation number 1, item number 1, and was affirmed as a final order on January 23, 2017, with respect to a workplace located at 5544 Ansel Drive Colorado Springs, CO. Cobra Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1004940, citation number 1, item number 1, and was affirmed as a final order on December 24, 2014, with respect to a workplace located at 630 N Tejon, Colorado Springs, CO.
