29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)  Cobra Roofing, Inc.  at 2750 Vista Pkwy, Erie, CO 80516:  On and preceding 5/10/19, the employer did not provide protective helmets where there was a possible danger of head injury from impact, or from falling or flying objects. Employees were re-roofing a commercial building golf clubhouse. The employees were throwing shingles, nails, and roofing materials to the ground and/or lower level while other employees were on the lower level or ground. The employer had not provided protective helmets. This condition exposed approximately 4 employees to struck-by hazards.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) Cobra Roofing, Inc.  at 2750 Vista Pkwy, Erie, CO 80516:  On and preceding 5/10/19,  the employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles. Employees were re-roofing a commercial building golf clubhouse. The employees were throwing shingles, nails, and roofing materials to the ground and/or lower level while other employees were on the lower level or ground. Employees were using pneumatic nail guns while shingling, and hammers to remove or replace roofing materials. The employer did not ensure that each affected employee uses appropriate eye or face protection. This condition exposed 8 employees to approximate struck-by hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  (a)  Cobra Roofing, Inc.  at 2750 Vista Pkwy, Erie, CO 80516:  On and preceding 5/10/19, the employer did not protect employees from injury in that employees accessed the roof using portable extension ladders, and 3 out of 4 portable ladders that did not extend at least 3 feet above the upper landing surface being accessed, and were not secured. This condition exposed 8 employees to approximate 10 foot fall hazards.

(a)  Cobra Roofing, Inc.  at 2750 Vista Pkwy, Erie, CO 80516:  On and preceding 5/10/19, the employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways.  Employees were re-roofing a commercial building golf clubhouse.  The employees accessed the roof levels with extension ladders.  The employer had not provided employees ladder training. This condition exposed 8 employees to approximate 10 foot and 40 foot fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  Cobra Roofing, Inc. at 2750 Vista Pkwy, Erie, CO 80516:  On and preceding 5/10/19, the employer did not protect employees from injury in that employees re-roofing multifamily homes were working without the use of or not utilizing fall protection. This condition exposed 40 employees to approximate 8.6 foot and 20.8 foot fall hazards.  Cobra Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1342687, citation number 1, item number 1, and was affirmed as a final order on December 28, 2018, with respect to a workplace located at 5080 S Irving St. Englewood, CO.  Cobra Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1189578, citation number 1, item number 1, and was affirmed as a final order on January 23, 2017, with respect to a workplace located at 5544 Ansel Drive Colorado Springs, CO.  Cobra Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1004940, citation number 1, item number 1, and was affirmed as a final order on December 24, 2014, with respect to a workplace located at 630 N Tejon, Colorado Springs, CO.
