OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to (description of hazard: (a) Pressure Test Area, 31 Locust Road, Ipswich, MA 01938 On or about 5/13/19 a box that was undergoing leak testing with a compressed gas exploded, distributing the pieces of the box outwards, striking an employee. The vessel was not outfitted with a pressure relief device per the 1968 ASME Boiler and Pressure Vessel Code. Among other methods, one feasible and acceptable abatement method to correct this hazard is: 1) Ensure parts undergoing testing are rated for accepting pressurized gasses, and are outfitted with adequate pressure relief devices. 2) Conduct leak testing of parts using existing leak testing equipment and protocols that do not require the part to be pressurized.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.: 31 Locust Road, Ipswich, MA 01938. Leak testing department. a) On or about May 13, 2019, a employees were exposed to struck by and pressure hazards from a compressed air gun that was found to have pressure greater than 30 p.s.i. at the nozzle.
