29 CFR 1926.501(b)(4)(ii):Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: Middle of Roof: On or about 9/10/2021, the employer exposed employees, who were repairing exhaust vents, to fall hazards of 20.3 feet above ground level, in that covers were not utilized to protect employees from falling through skylights.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: Middle of Roof: On or about 9/10/2021, the employer exposed employees to fall hazards 20.3 feet above ground level, in that employees had not been trained to recognize fall hazards and the measures to mitigate them prior to performing roofing activities.
