29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  At the worksite located at 17247 South 71 Highway, Belton, Missouri 64012, Employees were exposed to serious fall hazards. Employees operating the sheer machine were not properly protected from an open pit which was located next to the machine. The open pit was measured at 52 inches.

The Clark C500-55 forklift was leaking grease and hydraulic fluid and needed repairs, but was not taken out of service until restored to safe operating condition.
