At the worksite located at 17247 South Highway 71, Belton, MO 64012, employees engaged in material handling activities were exposed to caught between and crushing hazards in that the employer did not ensure that employees who were operating powered industrial trucks in the workplace had successfully completed training consisting of a combination of formal instruction, practical training, and evaluation of the operator's performance in the workplace.

At the worksite located at 17247 South Highway 71, Belton, MO 64012, employees engaged in steel fabrication activities were exposed to electrical shock, caught between and crushing hazards in that the employer did not ensure powered industrial trucks that employees were operating in the worksite had been examined daily before putting into service. Employees were exposed to caught between and crushed by hazards while operating unexamined forklifts to move  material throughout the facility as part of their daily work activities.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  At the worksite located at 17247 South Highway 71, Belton, MO 64012, employees engaged in sawmill activities at the worksite located at 33643 MO-6 Brashear, Missouri 63533 were exposed to electrical shock and burn hazards in that the employer did not ensure that cords and cables were connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws. Instances of exposure include but are not limited to:  Instance 1: A orange extension cord was observed to have damage associated with inadequate strain relief, which was being used to charge the battery on the green Clark forklift.  Instance 2: the power cord on a welding machine was observed to have damage associated with inadequate strain relief, which was being used to power the welding machine used by employees.
