On or about May 14, 2019, at the above addressed jobsite, an employee conducting water installation in a trench five (5') feet, nine (9) inches, in depth did not have a safe means of egress. Mcdonagh Contractors, LLC, was previously cited for a violation of this Occupational Safety and Health Standard 1926.651(c)(2), which was contained in OSHA inspection number 1170111, citation number 3, item number 1 and was affirmed as a final order on November 03, 2017, with respect to a workplace located at 744 W. 29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. a. On or about May 14, 2019, at the above addressed jobsite, an employee was conducting water connecting operations in a trench measured at five (5) feet, nine (9) inches in depth. The trench was not provided with cave in protection, thereby exposing the employees to cave in hazards. Mcdonagh Contractors, LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.652(a)(1), which was contained in OSHA inspection number 1170111, citation number 2, item number 1 and was affirmed as a final order on November 03, 2017, with respect to a workplace located at 744 W. Buckingham Place, Chicago, IL. Mcdonagh Contractors, LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.625(a)(1), which was contained in OSHA inspection number 1117967, citation number 2, item number 1 and was affirmed as a final order on November 17, 2017, with respect to a workplace located at 3757 N.
