Throughout the Agency; on or about 14 May 2019 and times prior thereto, the employer did not provide a written hazard assessment certification of the workplace evaluated to address existing conditions that could pose actual or potential safety hazards. Personal protective equipment observed in the workplace included but was not limited to; hearing protection, safety glasses, face shields, fall restraint devices, hard hats and gloves. This condition exposed employees to unsafe work practices.
