29 CFR 1926.404(b)(1)(ii):  Where an assured equipment grounding program was not utilized, receptacles were not protected with ground fault circuit interrupters when on a two-wire, single- phase portable or vehicle-mounted generator rated more than 5kW, or where the circuit conductors of the generator were not insulated from the generator frame and all other grounded surfaces:      a)  On May 14, 2019, employees were provided and used a 120 Volt, Honda EU3000is Portable Generator with receptacles not protected with ground fault circuit interrupters to power equipment and tools onsite.    No abatement certification or documentation is required for this item.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:    a)  On May 14, 2019, employees were exposed to hazards of falling approximately 40 feet to the ground during the transfers between an elevated aerial lift platform and the Safespan under-the-bridge suspended platform.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.502(d)(20): The employer did not provide for prompt rescue of employees in the event of a fall or did not assure that employees were able to rescue themselves:  a)  On or about May 14, 2019, the employer did not implement and maintain a rescue plan in the event of a fall into a live highway traffic from the Safeway under-the-bridge platform.
