(a) On or about May 15, 2019, the employer did not ensure that employees installing shingles with a pneumatic nail gun were wearing eye protection, thereby exposing employees to eye injuries from flying particles.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     a.   On or about May 15, 2019, at the construction site located at 8820 N. Spruce Rd, River Hills, employees were performing roofing work on a two story residential home with a pitch of approximately 6 in 12 and up to approximately 20 feet 4 inches in height without being tied off to an anchor point, thereby exposing the employees to a fall hazard.

Spruce Rd, River Hills, employees were exposed to a fall hazard of approximately 20 feet 4 inches and had not been trained to recognize and minimize the fall hazards associated with their work.
