29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group: On or about May 16, 2019 and at times prior thereto: a. In the pallet inspection area of SMETCO #1 pallet sorting machine where employees were exposed to struck-by and caught-by hazards while working on and around the pallet sorting machine that was de-energize without being locked out by all group members doing maintenance or servicing work on the machine. b. In the pallet inspection area of SMETCO #2 pallet sorting machine where employees were exposed to struck-by and caught-by hazards while working on and around the pallet sorting machine that was de-energize without being locked out by all group members doing maintenance or servicing work on the machine.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about May 16, 2018 and at times prior thereto: a. In the pallet inspection area of SMETCO #1 pallet sorting machine where employees were exposed to caught-by hazards while working near rotating rollers on the pallet conveyor without a guard protect employees from the rotating rollers. b. In the pallet inspection area of SMETCO #2 pallet sorting machine where employees were exposed to caught-by hazards while working near rotating rollers on the pallet conveyor without a guard to protect employees from the rotating rollers. Greatwide Distribution Logistics, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1365408, citation number 1 item number 1 and was affirmed as a final order on February 11, 2019, with respect to a workplace located at 225 Portwall Suite 300, Houston, TX 77029.
