29 CFR 1910.147(f)(3)(ii)(C) When more than one crew, craft, department, etc. was involved, the employer did not assign overall job-associated lockout control responsibility to an authorized employee designated to coordinate affected work and ensure continuity of protection. On or about August 29, 2019 and at times prior thereto in the pallet inspection area of SMETCO #2 pallet sorting machine where employees were exposed to struck-by and caught-by hazards while working on and around the pallet sorting machine that was de-energize without being locked out by all group members doing maintenance or servicing work on the machine.
