29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Northwest corner of shop:  On or about May 17, 2019, employees were exposed to struck-by hazards from flying projectiles while drilling holes in print machine dies without a chip guard on the Skil 10' Benchtop Drill Press, Model 3320.

29 CFR 1910.157(e)(1): The employer did not provide for the inspection, maintenance and testing of all portable fire extinguishers in the workplace:  On or about May 17, 2019, and at times prior thereto, employees were exposed to fire hazards, as portable fire extinguishers throughout the shop were not subjected to an annual maintenance inspection.   Brenholb, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard  29 CFR 1910.157(e)(1), which was contained in OSHA inspection number 1012663, citation number 2, item number 2, and was affirmed as a final order on 4/28/2015, with respect to a workplace located at 1234 Triplett St., San Antonio, TX  78216.

29 CFR 1910.1200(e)(1): Employers had not developed, implemented, and maintained at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met:  On or about May 17, 2019, and at times prior thereto, employees were exposed to health hazards without developing a written hazard communication program to address Safety Data sheets, labeling, and training requirements for chemicals used in the workplace such as, but not limited to, Napa All-Purpose, Water-Miscible Blanket and Roller Wash.
