29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  Along the production floor area, on or about December 09, 2014, and at times prior thereto, the following fixed power source enclosures, had unused openings along the bottom and sides of the electrical distribution panel boxes that were not effectively closed:   a) Along the center south side of the production floor, the fixed power enclosure Siemens Catalog Number G1224ML3200CU controlling the electrical power various outlets, lighting units.     b) Along the center south side of the production floor, the fixed power enclosure EATON Cutler-Hammer Cat. No. 3BRSF150 controlling the printing machine did not have a dead front panel.

29 CFR 1910.157(e)(1): The employer was not responsible for the inspection, maintenance and testing of all portable fire extinguishers in the workplace:  Throughout the production floor, on or about December 09, 2014, and times prior thereto, portable fire extinguishers were not subjected to an annual maintenance inspection, exposing employees to fire hazards, at the following locations:  a) Along the south area of the production floor, along the east end of the Heidelberg Speedmaster printing machine, a fire extinguisher had the inspection tag dated February 2013.  b) Along the west exterior entry door to the production floor, next to the electrical charging station for the forklift, afire extinguisher had the inspection tag dated February 2013.  c) Along the north interior wall area of the production floor, along the tool repair station, a fire extinguisher had the inspection tag dated February 2013.
