On or about May 14, 2019, and at times prior, employees were potentially exposed to crushing and struck-by hazards while working around Rotary auto lifts, in that the lift was not periodically inspected by qualified inspectors at least annually to ensure that they were properly maintained and functional.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:   On or about May 14, 2019, employees were exposed to a struck-by hazard when operating a DeWalt 8" grinder to deburr metal parts without the tongue guard adjusted to less than one fourth inch.     The U.S. Department of Interior was previously cited  a violation of this occupational safety and health standard 1910.215(b)(9), which contained in OSHA inspection number 1165884, citation 1 item 2 and was affirmed as a final order date on Oct 27, 2016 and was abated.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:   On or before 14 May, 2019, damaged Alkota 3158 electric hot water pressure flexible cord was not removed from service where the power cord strain relief had pulled away from the end plug fitting thereby exposing internal conductors.   The U.S. Department of Interior was previously cited  a violation of this occupational safety and health standard 1910.305(g)(2)(iii), which contained in OSHA inspection number 1165884, citation 1 item 4 and was affirmed as a final order date on Oct 27, 2016 and was abated.
