29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  Press 4, Printing Unit 3, Interprint, 101 Central Berkshire Blvd, Pittsfield, Massachusetts; On or about May 15, 2019 an employee was exposed to caught in and crushing hazards while performing cleaning of the gravure cylinder and ink pan without locking out the machine or providing alternative measures of effective protection.
