29 CFR 1904.29(b)(3): Each recordable injury or illnesses was not entered on the OSHA 300 log and/or incident report (OSHA Form 300A or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred:  785 Goddard Court, Alpharetta, GA (Worksite) - The employer is not entering recordable injuries and illnesses recordable onto the OSHA 300 logs within seven calendar days but is recording them at the end of the year for calendar year 2019. The first report of injury or illness which serves as an equivalent form was not filled out correctly and/or missing pertinent information; the following was not recorded onto the OSHA-300 logs,  on or about May 22, 2019.     a.  On or about January 15, 2019, Marketing & Proposal Manager - an employee fell onto a metal table injuring his left shoulder and upper arm.  This injury resulted in the employee receiving physical therapy and return to work with restrictions, per physician orders.     b.  On or about January 23, 2019, Quality Control Lead - an employee tripped and fell over the mat at the work station, injuring his right ankle and left knee.  This injury resulted in the employee receiving physical therapy and return to work with restrictions, per physician orders.    c.  On or about March 15, 2019,  2nd Shift Team Lead - an employee dislocated his shoulder.  This injury resulted in the employee resulted in the employee receiving return to work with restrictions, per physician orders.
