On or about May 22, 2019, at a new construction residential home worksite, an employee lifting siding materials from the ground level, to three employees who worked above him, did not have head protection in place and was exposed to stuck-by hazards from falling objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.  At new construction, residential home worksite, employees working at elevations of six feet or more above the lower level are not being protected from fall hazards by using fall protection systems: The employer fails to assure effective protection against falls as required by the standard, as demonstrated on May 22, 2019, when two employees installing framing materials on the second level of a new residential home located at 1889 Ponte Drive, Panama City Beach, Florida 32407, are not protected from a 19-foot, 6-inch fall hazard by the use of a personal fall protection systems.  First Prime Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number1319995, citation number 1, item number 1 and was affirmed as a final order on August 8, 2018, with respect to a workplace located at  Rainier Lane, Lot 21 - Seagrove Beach, Santa Rosa Beach, Florida 32459.

On or about May 22, 2019, at a new construction residential home worksite, an employee operated a rough terrain forklift without first receiving training and evaluation to ensure the employee was competent to operate the powered industrial truck safely, exposing that employee, and other employees at the worksite to stuck-by and crushed-by hazards.
