29 CFR 1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least four times the maximum intended load applied or transmitted to it: Cross Country Construction, LLC - CVA Oakland East Hub facility - 1007 County Rd O - Oakland, Nebraska 68045 - Employer is failing to ensure that a single-pole scaffold and its components could support its own weight and at least four times the maximum intended load applied or transmitted to it. On or about May 22, 2019, three employees were hospitalized from injuries suffered from the center pole scaffold collapse during grain bin construction.

29 CFR 1926.451(c)(3): Supported scaffold poles, legs, post, frames, and/or uprights were not plumb and braced to prevent swaying and displacement: Cross Country Construction, LLC - CVA Oakland East Hub facility - 1007 County Rd O - Oakland, Nebraska 68045 - Employer is failing to ensure that a single-pole scaffold was properly braced to prevent displacement. On or about May 22, 2019, three employees were hospitalized from injuries suffered from a center pole scaffold, inadequately braced with guy wires, that collapsed during grain bin construction.
