29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: a.) Meadville New Products, Inc., Grinding area, on or about May 23rd, 2019 and times prior thereto: Employees utilized a Sears and Roebuck brand (Serial #808250614) bench grinder for grinding plastics parts. The right side of the bench grinders work rest was approximately 3/4 of an inch from the grinding wheel/stone. The work rest had not been adjusted as the wheel was worn.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a.) Meadville New Products, Inc., Grinding area, on or about May 23rd, 2019 and times prior thereto: Employees utilized a Sears and Roebuck brand (Serial #808250614) bench grinder for grinding plastics parts. The right side of the Sears and Roebuck brand grinder was not equipped with a proper functioning tongue guard. The opening between the wheel and the end of the safety guard was measured at approximately 1 inch.
