29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  On or about May 23 2019, at 4720 Portofino Way, West Palm Beach, FL 33409, the employer's safety and health program did not include any procedure regarding frequent and regular inspections of the jobsite while employees installed a new roof on a residential structure and were not using any means of conventional fall protection system.

On or about May 23rd, 2019, at 4720 Portofino Way, West Palm Beach, FL 33409, the employer failed to instruct employees on the recognition and avoidance of fall related hazards prior to installing a new residential roof with a pitch of 4:12, at a height of 43 feet from ground level and not using a means of conventional fall protection system.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:   a. On or about May 23rd, 2019, at 4720 Portofino Way, West Palm Beach, FL 33409, employees were exposed to a fall hazard of 43 feet to ground level while installing a new residential roof with a 4:12 pitch slope and not attaching life lines to harnesses and to anchor points.     REPEAT: Jeff Albert Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), or its equivalent standard, which was contained in OSHA inspection number 1133346, citation number 01, item number 001, with respect to a workplace located at 424 Luna Bella Lanevenetian Bay Apartments, New Smyrna Beach, FL 32170 and was affirmed as a final order on 06/21/2016.  REPEAT: Jeff Albert Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), or its equivalent standard, which was contained in OSHA inspection number 1199110, citation number 02, item number 001, with respect to a workplace located at 4917 NW 55th Place Tamarac, FL 33319 and was affirmed as a final order on 01/20/2017.    REPEAT: Jeff Albert Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(11), or its equivalent standard, which was contained in OSHA inspection number 1144283, citation number 01, item number 001, with respect to a workplace located at 11301 NW 18th Street, Plantation, FL 33323 and was affirmed as a final order on 08/09/2016.
