Employee were exposed to the Yale Propane forklift E #13, and Yale Propane Forklift # E76, both did not have a nonslip surface on the brake pedal and were not taken out of service until they were repaired.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Employees were exposed to amputation hazards. Employees working in the Fab Area were exposed to the Wilton VSG Twenty Model No. 2025, Asset F330 Drill Press which did not have a chuck guard.  (b) Employees were exposed to amputation hazards. Employees working in the Fab Area were exposed to the Wilton VSG 20 Pine Drill Asset F362 which did not have a chuck guard.  (c) Employees were exposed to amputation hazards.  Employees working in the PVC Fab Area were exposed to the Accurshear which the guard did not adequately guard the hold downs, and the shear blade.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  Employees were exposed to struck by hazards from the grinding wheel.  Employees were using the Craftsman Bench Grinder in the Mold Maintenance Department. The left wheel facing the the bench grinder did not have the work rest adjusted to an 1/8 inch or less.

The Federal F8 Asset # F343 100 ton mechanical power press had an opening in the guard in the right side above where the material is being fed into the press.  It had a large enough hole that it did not prevent entry of hands or fingers from reaching through the guard.

The Iowa Machinery Supply Company vertical band saw in the Maintenance Shop had a portion of the flywheel which was unguarded.  The flywheel was not fully guarded within seven feet of the floor.

29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided:    (a) Employees were exposed to electrical hazards.  The Johnson 60 Punch Press, Asset # F374, had a two hand control that had a missing knockout.    (b) Employees in the Mold Maintenance Department were exposed to electrical hazards.  The Siemens 240 electrical breaker next to the lathe had two open knockouts on the left side.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    (a) Employees were exposed to electrical hazards.  Employees were exposed to electrical wiring for a light attached to the Iowa Machinery Supply Company vertical bandsaw which was 110/120 volts which was connected with wire nuts and not guarded by a cabinet or enclosure.    (b) Employees were exposed to electrical hazards.  Employees were exposed to electrical wiring as the Dock light to the right of  Dock Door # 2 was hanging loose from the fixture and the fixture was broken exposing employees to live wiring which was 110/120 volts.    (c) Employees were exposed to electrical hazards.  Employees were exposed to electrical wiring as the Dock light by Dock Door # 3 was hanging loose from the fixture exposing employees to live wiring which was 110/120 volts.
