29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. On 5/23/19, at the Woodstock job site, the employer did not ensure scaffold employees were working from had a ladder to access the working level of the scaffold and instead employees were climbing the frames of the scaffolds for access.

Such activities were not performed only by experienced and trained employees selected for such work by the competent person. On 5/23/19, at the Woodstock job site, the employer did not ensure scaffolds employees were working from were inspected by a competent person prior to permitting employees to work from that scaffold.
