A) Steel-It LLC on or about May 30, 2019 at the address of 301 East Lorain, Ohio: The employer did not ensure employee's had donned and secured their personal fall arrest systems to an JLG Aerial Lift. The employee's installing steel flashing and roofing material to the awning where exposed to a fall of 12 feet.
